Case Study of an Innovative Discharge Planning Program
Abstract
I worked with a discharge planning program at the
Advocate Christ Medical Center in Oak Lawn Illinois. My
role was to ensure that patients had a plan prior to their
discharge from the hospital. If I encountered a patient that
had barriers to care such as not having insurance,
difficulties with transportation, or experiencing
homelessness, I was tasked with helping the patient to
overcome these barriers.
This SIP was done to look at discharge planning initiatives
in America and how they are a tool towards equitable
healthcare for all.
